There are four appointment options. When booking online please be sure you are choosing the correct option for you. If it is your first visit, you will choose New Patient Appointment. All appointments thereafter will be a Follow Up Appointment unless you discuss Tuina Adjustments or Transformational Bodywork appointments with your practitioner.
Note: Jake is only available for comprehensive Chinese Medicine/Integrative Movement Medicine care via house calls at this time. His full-time acupuncture practice is housed on the third floor of Essentia Health’s First Street Clinic in Duluth.
New patient appointment
This in-depth appointment allows patients to tell their story, express concerns, and ask questions. Plenty of time is reserved for diagnostic processes, treatment, and lifestyle guidance. For first time patients only. No perfumes or make-up. Wear loose fitting clothing. Reserve 90 minutes.
Follow up appointment
Most health issues require multiple visits and may include acupuncture, moxibustion, bodywork, lifestyle counseling, personalized exercise instruction, and herbal formula recommendation. This is for those who've already been in for an initial appointment. Reserve 60 minutes.
tuina adjustment
This swift appointment is for people who require daily tuina (Chinese Physiotherapy) or those who are crunched on time but need treatment. This is a great option for those on work breaks who need quick pain relief as well as those who need intensive daily support for internal organ health. Reserve 10-20 minutes.
transformational bodywork
Note: This is not an openly available option. Jake only occasionally offers bodywork specific sessions for certain patients who require deep physical and emotional unwinding. If you are interested or feel the need for this type of work, contact Jake directly. Reserve 75 minutes.

*Insurance is not accepted but superbills can be provided for possible insurance reimbursement upon request.

Referral Program
When your friends, your family, and your coworkers schedule, it is because you said something to them directly or indirectly that inspired them to improve their health. Jake believes in rewarding this sort of behavior because encouraging people you care about to improve their health is inherently a very good thing. For every person that you refer, you will receive one $5 voucher. For every five people you refer, you will receive a $50 VOUCHER. These may be used individually or saved up to be used cumulatively. Policies
Payment is expected at the time of service. If you are going to be late, please make every effort to contact Jake as soon as possible. If you arrive more than 20 minutes late, you may be assessed half the cost of your scheduled appointment. Appointments cancelled within 24 hours of a scheduled appointment time may be assessed the full cost of your scheduled appointment. Every effort is made to ensure clinical appointments begin promptly, but keep in mind unforseen clinical circumstances do come up that require extra time. You may expect to be notified if your appointment time is delayed for this reason.
